Course Completion Forms

Course completion forms (C12s) are now available for purchase directly from POST. Due to the difficulty with agencies purchasing C12s directly from Scantron, POST has purchased a large quantity and will make them available to individual agencies at our cost.

To place an order, click on the link to the order form below. Included therein are instructions regarding how the order form should be submitted.

Once an order has been placed, POST will provide an invoice to the agency. POST must receive payment on the invoice before shipping the C12s.
